Please present your insurance cards at the time of registration. This office participates in many of the new HMO/PPO and Managed Choice companies:

• Medicare
• Blue Shield of Kentucky,
• Aetna
• HealthStar
• PHCS
• USA Health Network
• America's Health Plan
• Healthcare Compare
• Commonwealth Healthcare Alliance with CHA Health

If your insurance coverage is through one of these plans, then you will be asked for any CO-pay or CO-insurance amounts due at the time of service. If your insurance is not a part of these plans, then you will be asked for the full balance at the time of service. We will be glad to assist you with any claim submissions as needed for your reimbursement. Cathy McKnight will be glad to answer any questions that you may have regarding your coverage when you present you health insurance information.
